Red Clover Side Effects: What to Know and Who Should Be Cautious

Red clover (Trifolium pratense) has a long history of traditional use and a research literature that has grown steadily over the last three decades. It also has a genuine set of cautions — and those deserve to be stated plainly rather than tucked into a footnote. This article covers what is actually known about red clover's safety: how well tolerated it has been in clinical studies, what health authorities have and have not concluded, who should be careful, and — the distinction that gets blurred most often — why the safety questions around an oral supplement are not the same as the safety questions around a cosmetic cream.

First, a distinction that changes everything

Almost every safety caution you will read about red clover — including the ones in this article — comes from research on oral supplements: capsules and tablets delivering isolated isoflavones, usually somewhere between 35 and 150 mg per day, absorbed into the bloodstream and circulated through the body.1 That is the exposure route the risk assessments were built around.

A cosmetic applied to the skin is a different proposition. It is a different dose, a different route and a different pattern of exposure, and the clinical literature on systemic effects simply was not designed to describe it.

On the regulatory side, Trifolium pratense flower extract is registered in the EU's CosIng cosmetic ingredient database, under reference number 59842.2 It is worth being precise about what that means, because the phrase gets abused in marketing: CosIng is an inventory of ingredients used in cosmetic products in Europe — not an approval list, not an endorsement, and no indication of whether a given formula works. What it does tell you is that this is a conventionally used cosmetic ingredient, and we are not aware of any restriction or prohibition attached to it of the kind the EU Cosmetics Regulation applies to restricted substances.

How well tolerated is red clover?

The honest headline is that the tolerability record is reassuring, within limits.

The US National Center for Complementary and Integrative Health, part of the National Institutes of Health, summarises the position this way: red clover extracts have been used in clinical studies for as long as two years with apparent safety, and they appear to be well tolerated.3 Two years is a meaningful stretch of observation for a botanical supplement — a good deal longer than most.

Two qualifications belong alongside it. First, "well tolerated in trials" is not the same as "safe for everyone in every circumstance": trial participants are screened, and the people at highest theoretical risk are frequently the ones screened out. We will come back to that, because it matters more than it sounds. Second, tolerability is a separate question from effectiveness. The same NCCIH review notes that studies of red clover for hot flushes have produced inconsistent results, with some carrying a high risk of bias, and that findings on bone density have likewise been inconsistent.3 Being well tolerated is not evidence of working.

What side effects have been reported

Red clover has not been associated with a characteristic pattern of serious adverse effects in the clinical literature reviewed to date.3 Where problems have surfaced, they have tended to appear as isolated case reports rather than consistent trial findings — and case reports, by their nature, establish that something happened once, not that a product caused it.

An integrative medicine monograph compiled at Memorial Sloan Kettering Cancer Center records two such reports: an episode of subarachnoid haemorrhage in a woman taking a multi-herb supplement that contained red clover alongside dong quai and Siberian ginseng, and an episode of severe vomiting and epigastric pain in someone using red clover concurrently with methotrexate injections.4 In the first case the product contained several botanicals, so red clover cannot be singled out as the cause.

The more substantive safety conversation is not about common side effects at all. It is about specific populations and specific medicines.

Who should be cautious

Pregnancy and breastfeeding

The NCCIH states that red clover supplements may be unsafe during pregnancy or while breastfeeding.3 This is the clearest and least equivocal of the cautions, and it does not require a risk-benefit debate: red clover supplements are not appropriate during pregnancy or breastfeeding.

A personal history of estrogen-sensitive cancer

Red clover's isoflavones — chiefly biochanin A and formononetin — are phytoestrogens, meaning their structure resembles the body's own estrogen closely enough to interact weakly with estrogen receptors. That is precisely why they have been studied in menopause, and it is also why they warrant caution in hormone-sensitive contexts.

The Memorial Sloan Kettering monograph is direct about it, advising that people with breast cancer avoid red clover on the basis of its potential to stimulate proliferation of estrogen receptor-positive breast cancer cells.4 The same caution is conventionally extended to other hormone-sensitive conditions, including uterine and ovarian cancers, endometriosis and uterine fibroids. Anyone taking anticoagulants, antiplatelet drugs or NSAIDs

Red clover is not only isoflavones. The genus contains a broader mix of compounds, and the phytochemistry varies considerably between species and preparations.5 Among the constituents relevant here are coumarin derivatives — and coumarins are structurally related to the chemistry that anticoagulant drugs work through.

A review of herb-drug interactions with analgesics named red clover among the coumarin-containing herbal supplements with the potential to interact with non-steroidal anti-inflammatory drugs, aspirin included, in a way that could increase the risk of bleeding.6 The Memorial Sloan Kettering monograph makes the parallel point for anticoagulants and antiplatelet agents, noting that red clover may enhance their effects.4

It is worth being calibrated about the strength of this evidence: the concern is grounded in chemistry and in expert pharmacological review rather than in a body of controlled human interaction trials. It is a well-reasoned caution, not a documented high-frequency event. But for anyone on warfarin, on antiplatelet therapy or taking NSAIDs regularly, it is a real reason to check first. Children

Red clover supplementation is not advised for children. There is no meaningful safety evidence base in this group, and the phytoestrogenic activity is reason enough for caution.

What the EFSA opinion did — and did not — cover

The most substantial regulatory assessment of isoflavone supplements is a 2015 scientific opinion from the European Food Safety Authority's Panel on Food Additives and Nutrient Sources added to Food, produced at the request of the German Federal Institute for Risk Assessment.1

Its findings were broadly reassuring. Reviewing the human evidence, the Panel reported that the data did not support the hypothesis of an increased risk of breast cancer, and found no effect on mammographic density or on the proliferation marker Ki-67. It found no effect on endometrial thickness or on histopathological changes in the uterus up to 30 months of supplementation at 150 mg per day of soy isoflavones — although after 60 months some non-malignant histopathological changes were reported. Thyroid hormone levels were not altered by isoflavone intake from supplements.1

Now the part that is routinely dropped when this opinion gets quoted. The assessment was deliberately narrow, and its conclusions do not generalise beyond its scope. The Panel examined three organs — mammary gland, uterus and thyroid — in one population: peri- and post-menopausal women. It stated explicitly that the assessment addressed specific adverse effects in a pre-defined population and was not an overall evaluation of isoflavones in food supplements.1

And there is a further limitation that matters enormously for the group most likely to be worried. The Panel noted that in the majority of the human intervention studies it reviewed, a current diagnosis or history of breast cancer was typically listed among the exclusion criteria — meaning those sub-populations are likely to be under-represented in the assessment.1 In plain terms: EFSA's reassuring conclusions were largely drawn from studies of women without a cancer history, and cannot be read as clearance for women who have one. That is exactly why the caution in the previous section stands despite the favourable headline.

There is no established safe upper intake level

People often ask what the maximum safe daily dose of red clover isoflavones is. There is no authoritative answer, and it is better to say so than to invent one.

The EFSA Panel concluded that it was not possible to derive a single health-based guidance value or a safe intake level for food supplements containing isoflavones. Instead, it suggested that the doses and durations used in the intervention studies could serve as guidance for a dose and duration at which no effect was observed in the three organs assessed.1 That is a meaningfully weaker statement than an upper limit — it is a description of what has been studied, not a threshold that has been validated as safe.

Part of the reason is that "red clover extract" is not one thing. Isoflavone content and profile vary with genetics, growing conditions, harvest and extraction method. The EFSA Panel itself noted that inconsistent reporting of isoflavone content across studies made a formal meta-analysis unfeasible, and that other bioactive components — coumestrol among them — can be present in red clover extracts.1 Two products labelled identically can differ substantially in what they deliver.

When to talk to a doctor

Speak with a healthcare professional before taking red clover internally if you are pregnant, trying to conceive or breastfeeding; if you have a current or previous diagnosis of a hormone-sensitive condition, including breast, uterine or ovarian cancer, endometriosis or fibroids; if you take anticoagulant, antiplatelet or regular NSAID medication; if you take hormone therapy, hormonal contraception or an anti-estrogen medicine such as tamoxifen or an aromatase inhibitor; if you have thyroid disease or take thyroid medication; if you have surgery scheduled; or if you take prescription medication of any kind. The NCCIH's general guidance is worth repeating without embellishment: if you take any medicine, talk to your healthcare provider before using a herbal product.3

How much red clover is too much?

Anyone with a personal history of estrogen-sensitive cancer should discuss red clover with their doctor rather than relying on that conclusion.

Can I take red clover with my medication?

Not without asking first. The most discussed concerns involve anticoagulant, antiplatelet and NSAID medicines, because red clover contains coumarin derivatives, and hormone-related medicines such as tamoxifen.
